Our Pharmacist Conduct a Med Review on Each Patient Every Month

Pharmacies are communication epicenters. Information is constantly flowing into a pharmacy: new prescriptions, refills, titrations and  discontinuations of medications. To ensure all communication was received, recorded properly, and acted upon Meridian Meds has implemented a unique practice. We electronically compare every detail of a patients medication record to the previous month. If even a comma has changed we are alerted to the change. This process highlights dose changes, new medications, discontinued medications or simply a change of dosing times. Our pharmacists review the changes and follow up on questionable items. This process ensures continuity of medication and dosing practices from month to month.
